Our team of nurses in Yishun Community Hospital is committed to provide holistic care for our patients with Dementia. We adopt a person-centered care approach which tailors the needs of each individual patient respectively. This involves managing and caring for our patient with forgetfulness and cognitive problems as well as coaching for caregiver.
We provide various healthcare services such as:
- Behavioral management to help our caregiver better manages patient with behavior problems like repetitive behavior, agitation and angry behavior.
- Group therapy and Group activity to improve and maintain patient’s well-being and to build up rapport with their caregiver.
- Caregiver training on nursing related skills, home exercise to help patient and proper transferring technique so that caregiver are confident to manage patient at home.
Together with other healthcare professionals such as the medical team, physiotherapists, occupational therapists, speech therapists, dietitians, pharmacists and medical social workers, we collaborate closely as a team to facilitate family members in caring for their loves one. We advocate and energies patients, family and carers with relevant knowledge that is viable in their day to day care.
Our main aim is to empower families and their loved ones to cope with their condition out of the hospital and be able to rehabilitate back to the community.
