NURSING

Our team of nurses in Yishun Community Hospital is committed to provide holistic care for our patients with Dementia. We adopt a person-centered care approach which tailors the needs of each individual patient respectively. This involves managing and caring for our patient with forgetfulness and cognitive problems as well as coaching for caregiver.

We provide various healthcare services such as:

  • Behavioral Management
  • Group therapy and Group activity to improve and maintain patient’s well-being and to build up rapport with their caregiver
  • Caregiver Training

Together with other healthcare professionals, we collaborate closely as a team to facilitate family members in caring for their loved ones. We advocate and energies patients, family and carers with relevant knowledge that is viable in their day to day care.

Our main aim is to empower families and their loved ones to cope with their condition out of the hospital and be able to rehabilitate back to the community.

YCH palliative care service is a unit that provides efficient, cost effective specialized care to patients with advanced chronic illness or terminal disease.

Our palliative care team includes palliative trained physicians & nurses who work together with allied healthcare professionals to support and journey the patient and family with the goal of improving the quality of life throughout the course of the serious illness.

Our services include:

  • Symptoms Management such as pain and difficulty in breathing
  • Advanced Care Planning
  • Prepare for End-of-Life Care
  • Provide psychological, emotional and spiritual support.
  • Understand and support patients on their goals and preferences for care and treatment.
  • Palliative rehabilitation to improve their functions to the optimal.

Our rehabilitation nurses are committed to provide individualized care to patients. We aim to help patient to regain strength and maximize functional independence. Our nursing team works closely with other healthcare professionals in every phase of patient rehabilitation process. We also coordinate educational activities and use appropriate resources to develop and implement an individualized teaching and discharge plan with the patient and family during his or her stay in the hospital. We provide services not limited to stroke survivals, hip fracture patients and amputees. We design and implement treatment strategies to promote overall health and assist patient adapting to an altered lifestyle.

Rehabilitation services include:

  1. Caregiver Training
  2. Provide education on disease process
  3. Provide education and training on self-care
  4. Perform activities that maintain and restore function and prevent complications

YCH wound care service provides individualized care to patients with acute to chronic wound or any difficult to heal wound. Our specialty wound care nurse works collaboratively with multidisciplinary team which includes the combinations of expertise of internal medicine and infectious disease specialist; vascular, orthopaedic and plastic surgeon and also podiatrist, occupational therapist, physiotherapist, dietician, social worker and pharmacist.

Types of wound care services:

  • Wound Debridement
  • Negative Pressure Wound Therapy (NPWT)
  • Ostomy Care
  • Compression therapy
  • Biological therapist dressing

REHABILITATION SERVICES

Your quality of life matters to us. Our therapists from the Rehabilitation Department are committed to optimise your daily activities and quality of life as soon as possible.

Sessions are conducted on individual and group bases to deliver a wide range of holistic services centered on your needs.

These sessions may include:

  • Occupational Therapy to improve your independence and reintegrate you into community
  • Physiotherapy to restore your mobility, function and independence
  • Speech Therapy to treat communication and swallowing disorders

Each patient’s progress will be regularly reviewed and discharge planning conducted with the patient and/or family members. The patient and carer are encouraged to be active participants in the rehabilitation process.

MEDICAL SOCIAL SERVICES

Our medical social workers provide social and psychosocial support to patients and families to cope more effectively with their illness or challenging life circumstances.

Our range of services includes:

  • Care planning for persons with illness and disability
  • Counselling and therapy
  • Crisis management and intervention
  • Information and referral services
  • Financial assistance

NUTRITION AND DIETETICS

"Let food be thy medicine and medicine be thy food".

We believe that balanced and nutritious food is a very important component of treatment. As part of the clinical team, our dietitians assess your nutritional needs and recommend appropriate diets. Our dietitians work very closely with our chefs to provide meals that meet your needs and optimize your quality of life. We are committed to ensure that you are prepared to go back to the community by providing dietary advice.

Services are conducted in individual or group sessions. Topics include healthy diets, nutritional management of medical conditions and hands-on food preparation.

Services are charged separately and vary depending on your condition.

PHARMACY

Pharmacy aims to provide timely and cost-effective pharmaceutical care through remote and virtual care that is holistic and of high standards to our patients and customers from their and their carer's perspectives. We will provide seamless care through the continuum of care and integrate our patients back into the community.

We provide patient centered care through interviewing and engaging our patients and their caregivers during medication reconciliation. We set pharmacotherapy goals for individual patients. For selected groups of rehabilitative patients, we assess their abilities to self administer their medications during their stay in the hospital. We provide all patients with their personalised Patient Medication List and we use the list to educate and empower them and/or their caregivers on the names, indications and common side effects of their medications.
