Privacy Policy

GENERAL

​1Alexandra Health System Pte. Ltd, (“AHS”) manages:
1.1Khoo Teck Puat Hospital;
1.2Yishun Community Hospital; and
1.3Admiralty Medical Centre.
(each a “Healthcare Service Provider”)
2We, AHS, are committed to safeguarding your privacy. We will treat all personal data which you provide to us in the manner set out in this Policy. This Policy describes how we may collect, use, disclose, process and manage your personal data.
3By providing us with your personal data, you consent to our collection, use and disclosure (including transfer) of your personal data in accordance with this Policy. Please DO NOT provide any personal data to us if you do not accept this Policy.
4Information provided by organisations to us for business purposes is not considered as personal data. This Policy does not apply to business contact information.

AMENDMENT TO THIS POLICY

5We may amend this Policy from time to time – this version of the Policy was last updated as at 6 July​ 2017. The updated Policy will supersede earlier versions and will apply to personal data provided to us previously. The updated Policy will be made available upon request from us and on our website: click here. It is your responsibility to ensure that you keep up to date with any changes to this Policy. If you do not accept any amendment to the Policy, please contact our Data Protection Officer (see paragraph 28 below).

PERSONAL DATA

6 What is personal data? “Personal data” is data that can be used to ​identify a natural person. Examples of personal data include your name, mailing address, NRIC, passport or other identification number, telephone number(s), email address, photograph, video image, personal data of family members, medical records, education background, employment history, references and any other personally identifiable information relating to any individuals which you have provided us.
7 Voluntary provision of personal data. Your provision of personal data to us is voluntary. However, if you choose not to provide us with the personal data we require, it may not be possible for us to contact you, or provide services which you need from us.
8 Providing personal data belonging to others. If you provide the personal data of anyone other than yourself, you warrant that you have informed him/her of the purposes for which we are collecting his/her personal data and that he/she has consented to your disclosure of his/her personal data to us for those purposes.
9Accuracy and completeness of personal data. You must ensure that all personal data that you provide is true, accurate and complete.
10 Lack of consent or inaccurate personal data. You acknowledge that we are unable to provide you with products and/or services if you do not provide us with the necessary consent or if you fail to provide us with personal data that is true, accurate and complete.

​11 Anonymous browsing. Anonymity is maintained when you visit our websites. Our websites are not configured to gather any personally identifiable information from your computer, unless otherwise you personally and intentionally provide us with your personal information.
12 Use of cookies. We may make use of “cookies” on our websites to store and track information such as the number of users and their frequency of use, profiles of users and their online preferences. Cookies do not capture information which would personally identify you, but the information collected may be used to assist us in analysing the usage of our websites and to improve your online experience with us. You can disable the cookies by changing the setting on your browser. However, this may affect the functionality of the websites.
13 Web analytics. Web analytics is the term given to a method for collecting and assessing the behaviour of visitors to websites. This includes the analysis of traffic patterns in order, for example, to determine the frequency of visits to certain parts of a website, or to find out what information and services our visitors are most interested in. The web analytic services on our websites are provided by Google Analytics. This means that when you visit our websites, a cookie by Google Analytics will be stored on your computer, except when your browser settings do not allow for such cookies. Information about your behaviour at our websites will be collected and sent to Google Analytics for analysis for and on our behalf. Please note that if you have created an online profile at our websites and if you are logged on in this profile, a unique number identifying this profile is also being sent to Google Analytics in order to be able to match the web analytics data to this profile.

If you do not allow Google Analytics to collect and assess information about your behaviour at our websites, you can install the Google Analytics opt-out browser add-on. This add-on instructs the Google Analytics JavaScript to not send your website visit information to Google Analytics. The browser add-on is available for most modern browsers. The Google Analytics opt-out browser add-on does not prevent information from being sent to the website itself or in other ways to web analytics services.
14 Links to other websites. Our websites may contain links to other websites which are not owned or maintained by us. These links are provided only for your convenience. This Policy only applies to our websites. When visiting these third party websites, you should read their privacy policies.

COLLECTION OF PERSONAL DATA

15Generally, we may collect your personal data when:
15.1you seek medical treatment and services, healthcare and allied healthcare services at a Healthcare Service Provider;
15.2you visit a patient hospitalised in a Healthcare Service Provider;
15.3we conduct means testing to assess entitlement for subsidies and other financial assistance;
15.4you obtain medication from us;
15.5you submit an application form in order to register for any events or health screenings;
15.6you attend our events;
15.7you volunteer your services to us;
15.8you register for a user account in order to use and access certain online services offered through our websites;
15.9your image is captured through security cameras while you are within our premises;
15.10your image is captured on photographs or videos when you attend events organised by us;
15.11you submit an application for attachment, internship, scholarship, job, accreditation, sponsorship or any other relationship with us;
15.12you make a donation to us;
15.13you respond to our campaigns or initiatives;
15.14you interact with our employees or volunteers, for example, through our websites, social media platforms, face-to-face meetings, telephone calls, emails, fax and letters;
15.15you submit an enquiry, feedback or email through our websites or through other communication channels;
15.16you request that we contact you or request that you be included in an email or other mailing list;
15.17you enter into any agreement with us or provide any other documentation or information to us;
15.18we obtain information about you through the National Electronic Health Record system;
15.19we receive references from health care providers, public agencies, our related entities, strategic business partners and associates;
15.20we seek information from third parties about you in connection with any purposes identified in this Policy; and/or
15.21our security procedures require that you provide us with personal data prior to allowing you access to our premises and/or systems.

PURPOSES FOR COLLECTION, USE OR DISCLOSURE OF YOUR PERSONAL DATA

16 General purposes. Generally, we collect, use or disclose personal data for the following purposes:
16.1providing medical treatment and services, healthcare and allied healthcare services;
16.2managing and coordinating of medical treatment including follow-ups, step-down care and other continuity of care issues;
16.3inviting our patients to participate in clinical trials, clinical research or other studies relating to the services that we offer;
16.4contacting your family member(s) / next-of-kin(s) / donee(s) / deputy(s) for the purpose of providing them with your location within the Healthcare Service Provider’s premises (e.g. during visitation), medical updates, and seeking consent from them in emergency / incapacity situations;
16.5ensuring a patient’s health and safety (including advising on any product or drug recall);
16.6conducting medical reviews to better address your medical risks and improve patient care;
16.7conducting means testing to assess entitlement to subsidies and other financial assistance;
16.8reimbursement of healthcare providers for services provided;
16.9processing orders for and dispensing medication;
16.10processing appointments, bookings, admissions, transfers and discharge;
16.11processing and collecting payment for products, treatment and services;
16.12personalising your experiences at the touchpoints located within the Healthcare Service Provider’s premises;
16.13authenticating, operating, maintaining and managing your user account;
16.14to assist you with your submission of claims to your insurers and/or the Central Provident Fund;
16.15requesting for your feedback;
16.16to assist you with your enquiries, and respond to your feedback, complaints and requests;
16.17recording all telephone call conversations (including overseas calls) received to mediate complaints, for training purposes and for record keeping purposes;
16.18to keep you updated on campaigns, initiatives, programmes and events relating to our services;
16.19processing tax deductions for eligible donations by you;
16.20processing your application for attachment, internship, scholarship, job, accreditation, sponsorship or other relationship with us and assessing your suitability;
16.21managing the safety and security of our premises and events;
16.22managing crowds and patient queues;
16.23providing you with the services that you have requested;
16.24matching your personal data for any purposes identified in this Policy; and/or
16.25any other reasonable purposes related to the aforesaid.
17 Business purposes. We may also use and disclose personal data for purposes connected or relevant to our business, such as:
​17.1complying with our legal obligations and requirements;
17.2enforcing obligations owed to us;
17.3accounting, auditing, risk management and record keeping;
17.4carrying out research, planning and statistical analysis;
17.5reviewing healthcare policies and requirements;
17.6improving the quality of services provided by us;
17.7providing education and training for doctors and other medical, nursing or health professionals and students;
17.8using photographs and/or video recordings of you in our publications, websites and other communication channels; and/or
17.9​any other reasonable purposes related to the aforesaid.
18 Additional purposes. If you have consented, we may use personal data, from time to time, for additional purposes such as:
18.1to invite you to be a volunteer;
18.2to inform you of our initiatives and events; and/or
18.3to inform you of offers and promotions offered by our strategic business partners and associates.
19 Contacting you. When using your personal data to contact you for the above purposes, we may contact you via regular mail, fax, e-mail, SMS, telephone or any other means.

DISCLOSURE OF INFORMATION

20 Consent for disclosure to other health care providers. As we work closely with other health care providers and public agencies, you agree to allow us to share your personal data, financial information and medical records with them for the purpose of providing medical treatment and health care, and assessing entitlement to subsidies and other financial assistance. You also consent that we may make your personal data and medical records available on the National Electronic Health Record system, which enables health information to be shared throughout the Singapore’s healthcare system.
21 Consent for disclosure to others. If you have consented to our disclosure of your personal data to our related entities, strategic business partners and associates, we may disclose your personal data to them. They will use your personal data only for the purposes you have consented to.
22 Disclosure to service providers. You agree that we may disclose or share your personal data with third parties who provide necessary services to us, such as:
22.1doctors and other medical, nursing or health professionals who treat or have treated you, and their respective staff;
22.2service providers and data processors working on our behalf and providing services to us such as laboratory services, transportation services (including ambulance), accounting, payroll processing, telecommunications, information technology, hosting and maintenance services, webchat services, e-mail message services, market research, analysis services, storage and archival services, training, delivery services, handling of payment transactions, debt collecting services, customer support investigation services, organisation of events, marketing etc; and
22.3our consultants and professional advisors (such as accountants, lawyers, auditors).
23 Other permitted disclosure. We may also disclose your personal data as required or permitted by law. We may disclose your personal data to regulatory authorities, statutory bodies or public agencies for the purposes of complying with their respective requirements, policies and directives.

TRANSFER OF PERSONAL DATA OUT OF SINGAPORE

24 Transfer outside Singapore. You fully understand and unambiguously consent that we may transfer your personal data to any location outside of Singapore for the purposes set out above.
25 Protection of your personal data. If we transfer your personal data to a country or territory outside of Singapore, we will ensure that the recipient of the personal data provides a standard of protection that is comparable to the protection which your personal data enjoys under the laws of Singapore. Where these countries or territories do not have personal data protection laws which are comparable to that in Singapore, we will enter into legally enforceable agreements with the recipients to ensure that they protect your personal data to the same standard as required under the laws of Singapore.

PROTECTION OF PERSONAL DATA

26We will take reasonable precautions to safeguard your personal data.
27While reasonable precautions will be taken to ensure that the information you provide is protected against unauthorised or unintended access, we cannot be held responsible for unauthorised or unintended access that is beyond our control.

YOUR RIGHTS

28 Contact Data Protection Officer. If you wish to withdraw any consent you have given us at any time, or if you wish to correct or have access to your personal data held by us, or if you do not accept any amendment to this Policy, please contact our Data Protection Officer at ​[email protected].
29 Fee for access. We may charge you a fee for responding to your request for access to your personal data held by us, or for information about the ways in which we have (or may have) used your personal data in the one-year period preceding your request. If a fee is to be charged, we will inform you of the amount beforehand and respond to your request after payment is received. We will endeavour to respond to your request within thirty (30) days, and if that is not possible, we will inform you of the time by which we will respond to you.
​30 Effect of withdrawal of consent. In many circumstances, we need to use your personal data in order for us to provide you with products or services which you require. If you do not provide us with the required personal data, or if you withdraw your consent to our use and/or disclosure of your personal data for these purposes, it may not be possible for us to continue to serve you or provide you with the products and services that you require.

GOVERNING LAW

31This Policy is governed by the laws of the Republic of Singapore. You agree to submit to the exclusive jurisdiction of the Courts of Singapore in any dispute relating to this Policy.
